Hyperkeratotic fissured hand and foot eczema

First, it should be made clear whether this form of eczema is caused or supported by atopy, contact allergens or occupational factors such as high exposure to mechanical work. In most cases, there is a strong individual disposition.

Therapy depends on the individual skin condition: in cases of bacterial or fungal superinfection, topical antiseptics/ antibiotics/ antifungals may be required. In cases of severe inflammation topical corticosteroids may be necessary. Hyperkeratotic lesions are treated with keratolytics such as salicylic acid, urea (up to 40%) and tar preparations. Skin care may be performed with urea (10-20%) and lactic acid-containing topicals. In severe cases of hyperkeratosis, retinoids such as acitretine 0.5-1.0 mg/kg bodyweight may be used systemically for a limited periof of time.
